By creating an account or using Lantern Chart (the "Service"), you ("you" or "the user") agree to these Terms of Service. If you do not agree, do not use the Service.
1. What Lantern Chart is — and is not
Lantern Chart is a documentation drafting and workflow assistant for hospice clinicians. It helps you turn your own spoken or typed input into draft documentation (such as visit narratives, interdisciplinary group summaries, and messages to physicians). Lantern Chart is not an electronic medical record (EMR) or a system of record; not a source of medical advice, clinical decision-making, or diagnosis; and not a substitute for your professional judgment, your employer's policies, or your agency's required systems. All output is a draft only.
2. Your responsibility as a clinician
You are solely responsible for reviewing, editing, verifying, and approving every piece of documentation the Service helps you create before it is used, entered into any medical record, or relied upon. You are responsible for the accuracy and completeness of your documentation and for all clinical decisions. Lantern Chart does not verify the clinical accuracy of any output.
3. No identifying patient information
The Service is not HIPAA-compliant and is not designed, intended, or authorized to receive, store, or process protected health information (PHI). You agree not to enter any information that identifies a patient — including names, dates of birth, addresses, medical record numbers, phone numbers, Social Security numbers, photographs, or any other identifying details. You may describe a visit using non-identifying information only (for example, initials or "the patient"). You are solely responsible for anything you enter, and for complying with HIPAA, your employer's policies, and all applicable laws.
4. Accounts
You must provide accurate information when creating an account, keep your password secure, and are responsible for activity under your account. Accounts are for a single individual. Notify us promptly of any unauthorized use.
5. Acceptable use
You agree not to: use the Service unlawfully or with anyone's real PHI; attempt to reverse-engineer, copy, resell, or disrupt the Service; misrepresent the Service's output as verified clinical documentation; or use the Service in any way that violates your employer's or agency's policies.
6. Subscriptions and billing
Some features may require a paid subscription. If you subscribe: we may offer a free trial you can cancel before it ends to avoid charges; paid plans renew automatically until you cancel; founding-member pricing, where offered, remains at its stated rate for the life of that subscription as described at purchase; you can cancel anytime through the billing portal, which stops future renewals; payments are processed by a third-party provider (Stripe); and except where required by law, fees are non-refundable. We may change pricing for new subscriptions with notice; changes do not affect a founding-member rate that was expressly locked in.
7. Intellectual property
We own the Service, its software, design, and branding. You may not copy or reuse them. You retain ownership of the content you enter and the drafts you generate; you are responsible for how you use them.
8. Beta and "as is"
The Service is provided on an "as is" and "as available" basis, including during any beta period. To the fullest extent permitted by law, we disclaim all warranties, express or implied, including fitness for a particular purpose, accuracy, and non-infringement. We do not warrant that output will be accurate, complete, or suitable for any clinical or regulatory purpose.
9. Limitation of liability
To the fullest extent permitted by law, Lantern Chart and its operator will not be liable for any indirect, incidental, special, or consequential damages, or for any loss arising from your use of, or reliance on, the Service or its output. Our total liability for any claim will not exceed the amount you paid us in the twelve months before the claim.
10. Indemnification
You agree to indemnify and hold harmless Lantern Chart and its operator from claims arising out of your use of the Service, your documentation, your entry of any prohibited information, or your violation of these Terms.
11. Termination
You may stop using the Service and delete your account at any time. We may suspend or terminate access if you violate these Terms or misuse the Service.
12. Changes to these Terms
We may update these Terms. If we make material changes, we will notify you (for example, in the app or by email). Continued use after changes take effect means you accept the updated Terms.
13. Governing law
These Terms are governed by the laws of the State of Texas, without regard to its conflict-of-laws rules.
